How do I enter my name when ordering a name tag?

I'd like to order a custom name tag from the GBFans shop. I added to shopping cart and went to check out, but there's nowhere to enter a name. How do I do this?

I believe there should be an option when you reach the check-out to add any special instructions, and it's in that spot where you can specify the name you want to have included on the custom nametag. Hope that helps.
